Nurse-Led Support

Every conversation is guided by real clinical experience. Our nurses understand how care teams communicate, which details may matter, where confusion can happen, and how to help families prepare for what comes next.


Patient Advocacy

We help people find and use their voice inside the healthcare system. We listen for concerns that remain unanswered, prepare questions, and help keep the patient’s needs, preferences, and dignity present in the conversation.


Care Navigation

We help people see the path ahead. We translate medical language, organize information across appointments and providers, and turn care instructions into manageable next steps.

Together, these services create a continuum of support. Some people need one focused conversation. Others need a deeper plan, ongoing guidance, or a nurse physically present during a hospital stay, appointment, or transition home. Easter’s Heart meets each person at the level of support their situation requires.

Why This Work Matters

People are often expected to navigate the hardest medical moments of their lives with limited time, unfamiliar language, and incomplete support.

A diagnosis can introduce a new world overnight. A hospital discharge can send a family home with pages of instructions and no clear sense of how the pieces fit together. A long-distance caregiver may have to make decisions based on rushed updates. A patient living alone may have no one beside them to listen, take notes, or ask the question that changes the conversation.

These moments can shape health, recovery, trust, and a family’s ability to care for one another. People deserve time to understand their options. They deserve questions answered in language they can use. They deserve support that respects their agency and helps them remain connected to their own care.

Meet Our Founder

Lucinda Shelby-Allen,
MSN, RN, PHN, CMSRN

Lucinda Shelby-Allen is a registered nurse, educator, leader, and lifelong caregiver with over 25 years of healthcare experience.

Her career spans medical-surgical and cardiac nursing, case management, clinical triage, quality improvement, patient transitions, nursing leadership, and simulation education. She has served as a charge nurse, case manager, nurse manager, quality nurse consultant, clinical triage nurse, instructor, and simulation education leader across respected Bay Area healthcare and educational institutions.

Lucinda holds a Master of Science in Nursing Education and brings more than 10 years of teaching experience to her work. She is a Registered Nurse, Public Health Nurse, and Certified Medical-Surgical Registered Nurse.

Her ability to connect bedside knowledge, systems thinking, and patient education is the clinical foundation of Easter’s Heart. Her way of caring gives the practice its spirit.



A LETTER FROM OUR FOUNDER

You deserve someone in your corner, pushing for you.

For 25 years, I have been a nurse. For just as long, I have been the person my family calls when someone we love is sick, in the hospital, preparing for surgery, or trying to understand what the doctor said.

I know how much information can arrive at once. I know how quickly a family can move from an ordinary day into a world of diagnoses, medications, specialists, and decisions. I also know that even the most capable person can feel unsure when the health of someone they love is at stake.

Over the years, I have watched patients and families struggle to understand what was happening and what they were supposed to do next. I have stepped in to ask questions, explain the language, connect the details, and make sure important concerns were heard. I have seen how the right support can change a family’s experience of care.

Easter’s Heart is named for my mother, Easter, whose love made room for everyone. She taught us to move toward people with warmth, courage, and generosity. I created this practice to carry that way of caring forward and make experienced nurse guidance available to more people when they need it most.

When you come to Easter’s Heart, you do not need to have the right words or a perfectly organized story. Tell us what is happening. Bring the instructions, the questions, the worry, and the details you are still trying to understand. We will begin there.
